Photocopy of Valid Business Permit or Certification from the Barangay or MAO for small scale farmer   3. Clear copy of OR/CR of vehicle to be used   4  Duly Accomplished Statement of Commitment and Commitment of Undertaking   5. Duly accomplished certification indicating the name of driver and helper of the vehicle to be used in applying for IATF-ID   6.
